Before you begin a request to establish an EPIC account, you must read this important information.

To establish an EPIC account, you must submit basic personal information, details about your medical education, details about your postgraduate medical education and licensure, if applicable, and documents to confirm your identity. The application should take approximately 15 minutes to complete. Before you begin, you must read the ECFMG Policies and Procedures Regarding Irregular Behavior and have the following available:

  • A scanned copy of the photograph page of your current, unexpired passport (see Confirming Your Identity with EPIC for complete guidelines);
  • A scanned copy of the page that contains your passport expiration date, if applicable;
  • A scanned copy of the section of your passport that is in Latin characters, if applicable;
  • A full-color photograph of yourself (see Confirming Your Identity with EPIC for complete guidelines)
  • Information about your medical education, including your medical school identification number (if applicable) and attendance and degree issue dates;
  • Your medical licensure information, including any identification numbers;
  • Your USMLE/ECFMG Identification Number (if you have one); and
  • Your ECFMG International Credentials Services (EICS) Identification Number (if you have one).

Important Note: If you are unable to answer all items in one session or if you leave the request for any reason, your request will not be saved and you will be required to begin a new request when you return. Therefore, we recommend that you gather all of the above information, including your scanned passport and photo, prior to beginning.

Once we have received and processed your completed request, including payment, we will e-mail you confirmation that your EPIC account has been established. This e-mail will include instructions on how to access your EPIC Portfolio to print and complete the EPIC Identification Form, which must be certified by a Consular Official, Notary Public, First Class Magistrate, or Commissioner of Oaths, and submitted to ECFMG. Once your completed form has been received at ECFMG, you will be able to begin adding medical credentials, including diplomas, transcripts, certificates of postgraduate training, and certificates of medical registration/licensure, to your EPIC portfolio.
